Identify the token, leader, network and time, then inspect the reason. Check relevant funds, permission, price protection and saved limits in that order. Changing every setting at once makes it harder to know why the next action behaved differently.
Worked example
A gas shortage needs gas on that chain
If a row names insufficient gas, a larger stablecoin balance on another chain does not address the same condition. Refresh the relevant chain data after funding rather than repeatedly creating the same copy.
